South Georgia Nurse Practitioner Charged with Falsification of Records

On August 9, 2017, Tonya Yawn-Lewis, of Helena, GA, was charged by information in U.S. District Court, Southern District of Georgia, with falsification of records with intent to impede proper administration of DOT. Yawn-Lewis is a licensed nurse practitioner, and at the time of the offense, was an approved medical examiner on FMCSA’s National Registry of Certified Medical Examiners.

DOT-OIG’s investigation revealed that Yawn-Lewis routinely signed DOT Medical Examiner’s Certificates and Medical Examination Reports for commercial driver’s license (CDL) holders without performing medical examinations on the drivers.

DOT-OIG is investigating this case with the Drug Enforcement Administration with assistance from FMCSA.
