Texas Man Sentenced for Falsifying a DOT Medical Form and Medical Card

On August 31, 2016, Michael Glenn Dairy, was sentenced to 24 months of probation in U.S. District Court, Abiliene, Texas, for falsifying a registered nurse’s (RN) signature on a DOT medical form and card. Dairy submitted the false document to gain employment as a driver at O-Tex Pumping in Midland, Texas. 
 
DOT-OIG’s investigation was initiated after O-Tex Pumping contacted the RN to obtain her national registry number for Dairy’s DOT medical form and medical card. The RN informed O-Tex Pumping that she had not signed the medical form or card. Dairy was subsequently interviewed by DOT-OIG agents and admitted he had falsified the medical form and card by using the RN’s signature from a previous physical examination. 
 
This investigation was conducted with assistance from the Big Spring, Texas, Police Department.
